Local Care Team Manager

Position Summary

The Local Care Team (LCT) Manager provides day-to-day management and coordination of the Local Care Team as prescribed by the Governorís Office of Children. The LCT Manager also provides community outreach, family navigation, and case management to support the LCT implementation for Baltimore City.

Essential Functions

  • Lead the growth and expansion of the Local Care Team activities in Baltimore City by engaging with appropriate partners and extending the reach of the LCT beyond its current capacity.
  • Manage the logistical functions of the LCT implementation by preparing meeting agendas, keeping detailed meeting minutes, tracking attendance, and providing other meeting logistics such as scheduling meeting rooms, distributing meeting information, etc.
  • Ensure that Plans of Care developed during Local Care Team meetings are enacted and adhered to during the service period. 
  • When necessary, provide family navigation support to individuals referred to the LCT.
  • Work with members of the Data & Evaluation team to provide accurate and timely reports to state-level interagency placement council.
  • With other members of the LCT, identify the needs of the community and implement thoughtful, meaningful responses through the LCT operations. 
  • Maintain a directory of community-based resources for distribution.
  • Provide training on the Local Care Team purpose and operations to new LCT members.
  • Conduct outreach to community-based groups to build engagement with the community and continue to build network of resources.
  • Follow organizational procurement policies and practices, ensuring that assigned initiative areas have needed supplies and materials in a timely fashion.
  • Complete a broad variety of administrative tasks including: completing expense reports, compiling monthly and quarterly reports; managing program expenses; distributing program materials; program administration; and other programmatic needs as necessary to ensure the implementation of the LCT.

Other Functions

  • Provide updates for assigned initiative areas, as appropriate, to internal communication news sources. 
  • Educate coworkers and other community members about the functions of the LCT and its role in the community when needed.
  • Perform other duties and special projects as assigned by the Senior Director of Collaboration.


Education & Experience Requirements

  • Commitment to working with a diverse team on implementing culturally inclusive practices and participating in dialogue on equity in the communities served. 
  • Minimum of 3 yearsí experience in case management, preferably with children and young adults. 
  • Bachelorís degree in social work, human services, or related field required, masterís degree preferred.
  • Experience working in a fast-paced, mission-driven environment and a genuine interest in human services.
  • Experience and interest in internal and external communications, project management and calendar management.
  • Strong organizational skills that reflect ability to perform and prioritize multiple tasks seamlessly with excellent attention to detail.
  • Strong interpersonal skills and the ability to build relationships with stakeholders, including staff, external partners, city officials, and elected officials.
  • Proficiency in Microsoft Office Suite.
  • Exceptional written and verbal communication skills.
  • Demonstrated proactive approaches to problem-solving with strong decision-making capability.
  • Proven ability to handle confidential information with discretion, be adaptable to various competing demands, and demonstrate the highest level of customer/client service and response.
  • Demonstrated ability to achieve high performance goals and meet deadlines in a fast-paced environment.
  • Forward looking thinker, who actively seeks opportunities and proposes solutions.
  • Must have a passion for the mission and vision of Family League.
  • Baltimore City residency preferred.

Additional Information
This position is eligible for a cell phone stipend in the amount of $60/month.
This position is not eligible for telework or compressed work week.
Annual salary is $55,000.
